Speaker:

it affects men of all ages and backgrounds. So let's get

Speaker:

Speaker:

resources from the show. See you on the inside.

Speaker:

Hello there, Modern Man. I'm Dr Anne Truong, a sexual health

Speaker:

medical doctor. In today's episode, I'm just going to talk

Speaker:

about something that I usually don't want you to do, but I want

Speaker:

to give you the facts so you are aware of what you're getting

Speaker:

into if you are taking either Viagra or tadalafil or generic

Speaker:

name for Viagra is sildenafil and generic name for Cialis is

Speaker:

tadalafil.

Speaker:

Now, as you know, I always want you to do natural approach to

Speaker:

sexual function, but sometimes you are taking this medication,

Speaker:

you want to know the difference between the two, and when to use

Speaker:

it. What's the difference and what are the potential side

Speaker:

effects that are not talked about. So Viagra is the brand

Speaker:

name and the generic name is called sildenafil, and Cialis is

Speaker:

the brand name and the generic name is tadalafil. They both are

Speaker:

what is called PDE-5 inhibitors, so they help in increasing

Speaker:

nitric oxide in penis, which then increases blood flow. But

Speaker:

it doesn't just work on the penis alone. It works in other

Speaker:

tissues, other organs as well, and that's why you have more

Speaker:

side effects as well, which we will discuss about and one of

Speaker:

the most common side effects is a blue haze in eyes for

Speaker:

sildenafil, for tadalafil or Cialis is muscle aches. But I

Speaker:

will talk about the common side effects later.

Speaker:

So for Cialis, which is the brand name, it lasts longer than

Speaker:

Viagra. Cialis remains effective for 36 hours, and Viagra

Speaker:

typically lasts between four to six hours. It starts to work or

Speaker:

the onset of action for Viagra is between 30 minutes to an hour

Speaker:

And Cialis 30 minutes to two hours. So what that means is

Speaker:

that if you take Viagra, you should think about having sexual

Speaker:

activity between 30 minutes to an hour, and Cialis, you have a

Speaker:

little bit more leeway between 30 minutes to two hours. So I

Speaker:

hear, commonly, my patient say, "Well, I take Cialis and it

Speaker:

didn't really work." Or sometimes it may just be that

Speaker:

you may be getting ready for a sexual activity too early, or

Speaker:

you didn't wait long enough, and in some men, it may take up to

Speaker:

two hours. That's why Cialis sometimes is recommended as

Speaker:

daily use. Now the dosage of daily use is a lot lower than on

Speaker:

demand use. The sildenafil or Viagra is usually not used as a

Speaker:

daily dose, but more used as an on demand dose at this time. And

Speaker:

the dosage combination options are that low dose for Cialis,

Speaker:

about 2.5 to 5 milligrams, while Viagra is often as needed, like

Speaker:

I said before. Now, one thing about Viagra is that it is

Speaker:

dependent upon what you eat. So if you eat a fatty meal right

Speaker:

before you take it, then absorption will be less versus

Speaker:

tadalafil is not really dependent upon what you eat, and

Speaker:

both these medications can cause headaches and flushing, but

Speaker:

particularly with Cialis, you'll start having like muscle aches

Speaker:

versus Viagra is more vision changes and dizziness. So due to

Speaker:

the longer duration, Cialis allow you to be a little bit

Speaker:

more spontaneous. That's why you take it daily. Now, the dosage

Speaker:

for daily Cialis between 2.5 to 5, sometimes 10 milligrams. You

Speaker:

can take it daily. It's also been helpful for also prostate.

Speaker:

And then Viagra usually is taken as needed, between 20

Speaker:

milligrams, 50 milligrams, up to 100 milligrams. And if you're

Speaker:

taking more than 100 milligrams, you shouldn't be taking Viagra

Speaker:

that much. Okay? And that's number two.

Speaker:

Let's talk about the side effects. Now, of course, if you

Speaker:

take the medication brand name, then it'll be more costly versus

Speaker:

if you're going to take a generic. Okay, let's talk about

Speaker:

side effects. One of the most common side effects, or Viagra,

Speaker:

is headache and flushing, and that's more common than Cialis.

Speaker:

And then you also have upset stomach, which is common for

Speaker:

both, nasal congestion or stuffy nose is very common for both as

Speaker:

well. Now versus Cialis, you have more muscle aches,

Speaker:

particularly in the shoulder, back, arms and legs, less common

Speaker:

with Viagra, but you can have that. Dizziness is more common

Speaker:

with Cialis than what Viagra. Vision changes is more common

Speaker:

with Viagra and less common with Cialis. Viagra more common with

Speaker:

priapism, or prolonged erection that is lasting more than four

Speaker:

hours. Now this is an emergency. If you have that you got to go

Speaker:

to the emergency room to get an injection to bring it down. It's

Speaker:

less common for Cialis. And then sudden vision loss is rare, but

Speaker:

it can be reported as a permanent in some cases as well,

Speaker:

in both Viagra and Cialis, hearing loss or decreased loss

Speaker:

of hearing, sometimes with tinnitus, which is ringing in

Speaker:

the ears, is prevalent in both cases. Uncommon, but it is

Speaker:

prevalent. Low blood pressure can happen in both Viagra and

Speaker:

Cialis. The low blood pressure, which then lead to dizziness,

Speaker:

fainting spells, and especially it doesn't mix well with

Speaker:

alcohol, and you can have chest pain, even heart attack and

Speaker:

irregular heartbeat with Viagra and Cialis, though it is rare,

Speaker:

but it could happen. You can even have swelling of the face,

Speaker:

lips and tongue, both these medication due to allergies.

Speaker:

So to summarize, there are some serious side effects, and there

Speaker:

are common side effects, but Viagra has side effects of

Speaker:

blurred vision and headache versus Cialis more muscle pain,

Speaker:

but you could have low blood pressure will cause you fainting

Speaker:

spells, which then lead to irregular heart rhythm as well.

Speaker:

I know that lot of men are on these medications. I want you to

Speaker:

know what medication does what, when to use it. There's another

Speaker:

question, can you take both of it at the same time? Shouldn't.

Speaker:

Try to be very cautious with it, because if you take both of it

Speaker:

at the same time, it's a double effect in lowering the blood

Speaker:

pressure, because while it increases blood flow to the

Speaker:

penis, it also lower blood pressure as well, and that's how

Speaker:

you have fainting spells and heart attacks. And if you are

Speaker:

taking it together, it should be compounded, and you should

Speaker:

follow seeing a doctor for monitoring, because usually we

Speaker:

can compound and put a little bit of sildenafil and a little

Speaker:

bit of tadalafil, along with some oxytocin together. And then

Speaker:

oxytocin help with getting you in the mood and also help with

Speaker:

the feeling of love.

Speaker:

Now we really want to point out something very, very important,

Speaker:

is that the Cialis and Viagra does not get you in the mood, it

Speaker:

does not increase arousal, it does not increase your libido at

Speaker:

all. All all it does is just increase blood flow, but you've

Speaker:

got to do the work in libido and arousal. If you don't, no

Speaker:

erection is going to happen. You can't just pop up here and go,

Speaker:

Okay, I'm ready. No, you've got to get in the mood. You've got

Speaker:

to get aroused. You got to start in the brain. I've always said

Speaker:

that erection starts in the brain, not in the penis. So

Speaker:

that's one thing to consider. You're taking the medication,

Speaker:

just know that it's not going to help you with arousal, which you

Speaker:

have to initiate in your brain, as in using the five senses that

Speaker:

you are given, your vision, your nose, your mouth, sound and

Speaker:

touch to get you aroused, and get you in the mood to get

Speaker:

things started. If nothing starts up here, guys, nothing

Speaker:

gonna go down below your belt, even with Viagra and Cialis.

Speaker:

Now, of course, if you have ED, the first thing you should do is

Speaker:

one, go get some blood work, find out whether you may have

Speaker:

diabetes, which is high blood sugar, high cholesterol, or even

Speaker:

high blood pressure, and also, how's your testosterone level?

Speaker:

Is there any type of inflammation going on? Number

Speaker:

two, look at your diet. What are you eating? You should be eating

Speaker:

clean food, like follow Mediterranean diet. Clean food

Speaker:

is just vegetables and less processed food. Don't eat

Speaker:

anything in a box or a bag. Eat what the earth gives you. Eat

Speaker:

less red meat. Once a week is okay, but eat more chicken,

Speaker:

pork, fish. That's it. It's not that complicated. And if you're

Speaker:

smoking, stop smoking, because smoking will just kill your

Speaker:

erection, period. And make sure you get at least seven hours of

Speaker:

sleep and make sure that you keep your weight under control,

Speaker:

keep your BMI less than 30. You can look up in Google what your

Speaker:

BMI by entering your height and your weight, and most

Speaker:

importantly, keep your mind active and know that the

Speaker:

erection starts in the brain. Now there are alternatives to

Speaker:

taking medication. There is a research article that came out

Speaker:

that says that diet and exercise, diets such as taking a

Speaker:

Mediterranean diet, and also doing exercise, just like

Speaker:

walking 30 minutes four times a week, is as effective, or more

Speaker:

effective than taking Viagra or Cialis. Just know that it is

Speaker:

more effective. So instead of popping that pill and having to

Speaker:

endure the side effects and then, because you take the pill,

Speaker:

you can really going to ignore why you have ED in the first

Speaker:

place. It's just a band aid. Because if you take the pill, it

Speaker:

may work out a few years, it may not work. Where will you be?

Speaker:

Because you can just quickly change it with your diet and

Speaker:

your exercise, and know that need to stop smoking. You need

Speaker:

to stop porn, you probably need to stop masturbating frequently

Speaker:

as well. Frequently, what I mean is more than probably twice a

Speaker:

day for masturbation.

Speaker:

Having said that, just know that there are alternative to

Speaker:

medication. I wanted to kind of bring this up because I got a

Speaker:

lot of questions regarding medication at this point and

Speaker:

that just know that diet and exercise is as effective as

Speaker:

Viagra and Cialis, and just know that you are not alone. We are

Speaker:
