All right, so let's dive into this. So let's just

Anne Truong:

talk about why should men care about sleep, and how does it

Anne Truong:

that relate to their erection?

Ram Verma:

Yeah, so sleep is very important for the erection

Ram Verma:

and ED. Sleep problem start first and then ED might come

Ram Verma:

later on. So try to check your sleep. If you are having

Ram Verma:

snoring, breathing pauses, having low oxygen when you

Ram Verma:

sleep, and your sleep is getting broken, so that disturb the

Ram Verma:

activity of the brain gland to secrete hormone. If sleep is

Ram Verma:

broken, those system get broken.

Anne Truong:

Okay, so let's just kind of break it up right here.

Anne Truong:

So how does sleep affect their sexual function? You mentioned

Anne Truong:

that sleep is broken and it affect their hormone. How does

Anne Truong:

that happen? What happened to the body that caused that?

Ram Verma:

So during sleep, like you know, people require to have

Ram Verma:

certain amount of deep sleep, REM sleep. So during the deep

Ram Verma:

sleep, the pituitary gland and some other like gland in the

Ram Verma:

body, they secrete hormone. But if someone has sleep apnea or

Ram Verma:

some sleep problem waking up so many times, they are not able to

Ram Verma:

maintain the rhythm of continuous deep sleep for

Ram Verma:

certain period of time to produce those hormones. So when

Ram Verma:

the sleep is broken, that system get broken. And when the glands

Ram Verma:

are not able to secrete the hormones, so the hormones level

Ram Verma:

get disturbed and that affect the ED.

Anne Truong:

Okay, so let's talk a little bit about sleep apnea.

Anne Truong:

What is it?

Ram Verma:

So sleep apnea is a condition in which people snore

Ram Verma:

and get breathing pauses during the night when they are

Ram Verma:

sleeping. And when the breathing pauses happen, their oxygen

Ram Verma:

level goes low. So when person is sleeping and in certain

Ram Verma:

stage, like deep sleep or REM sleep, and the oxygen level goes

Ram Verma:

low, then the brain get arousals. And in those moments

Ram Verma:

of arousal, the sleep get broken, and when it is broken,

Ram Verma:

then that affect the ability of body to produce those hormone at

Ram Verma:

certain level.

Anne Truong:

Okay, so how does the person know that they have

Anne Truong:

sleep apnea? Because when you're asleep, you're asleep, you don't

Anne Truong:

really know. So what are some of the things that they may notice

Anne Truong:

that, Hey, maybe I have sleep apnea. Because I know that,

Anne Truong:

well, let's backtrack that how common is sleep apnea in men?

Ram Verma:

So sleep apnea is very common in men, maybe 10% to

Ram Verma:

25% of men, they tend to have sleep apnea. Most of them are

Ram Verma:

undiagnosed. And the symptoms are like snoring, getting

Ram Verma:

breathing pauses when sleeping, stopping breathing, and frequent

Ram Verma:

awakening during the night, going to bathroom multiple time

Ram Verma:

during the night, and when waking up feeling tired.

Ram Verma:

Sometimes they feel more tired even they before going to bed.

Ram Verma:

And during the daytime, they are having difficulty staying awake

Ram Verma:

at work, focusing on their work, or when they are driving, they

Ram Verma:

feel sleepy. So those are some symptoms of sleep apnea.

Anne Truong:

Gotcha. And how long does it have to go on until

Anne Truong:

they start feeling those symptoms?

Ram Verma:

Like it takes, like, few years. So this problem

Ram Verma:

getting ignored, people are not aware about it, and that's why,

Ram Verma:

like no getting ignored, and then they develop those things.

Ram Verma:

And they develop like other other health condition, like

Ram Verma:

high blood pressure, diabetes, ED and some other like dementia,

Ram Verma:

depression, anxiety, headache, all those other other conditions

Ram Verma:

comes on later on.

Anne Truong:

Okay, so how does sleep apnea cause ED?

Ram Verma:

So sleep apnea causes breathing pauses in you know,

Ram Verma:

when they are sleeping and it disturb the quality of sleep.

Ram Verma:

And it fragments the sleep. People are not able to maintain

Ram Verma:

deep sleep or REM sleep, so their hormone level goes low.

Ram Verma:

And the other thing is also when they are sleeping, having

Ram Verma:

undiagnosed or untreated sleep apnea, so that also affects the

Ram Verma:

body's ability to heal during the sleep, and they are feeling

Ram Verma:

tired. So when they are not having energy and not having

Ram Verma:

enough hormones, that's how it affects their sexual health.

Anne Truong:

Gotcha, that's the connection there, it's really

Anne Truong:

the hormone. So it's affecting testosterone. It doesn't

Anne Truong:

necessarily affect blood flow, it just really testosterone more

Anne Truong:

of a hormonal impact?

Ram Verma:

Yeah. So there has been studies done. So it affects

Ram Verma:

hormone level, but it also affects some dysfunction in the

Ram Verma:

circulation system also. Their endothelial system, and other

Ram Verma:

other other things get also disturbed. So those are on

Ram Verma:

chemical level and inflammation in the circulation, in the vein

Ram Verma:

and arteries. So those also affect so. So that's how it

Ram Verma:

affects sexual health as well.

Anne Truong:

Gotcha. Okay, so sleep apnea is presented if

Anne Truong:

you're if you're sleeping, if you're tired, that's one of the

Anne Truong:

main symptoms, right? And what are the risk factor that

Anne Truong:

predispose somebody to have sleep apnea?

Ram Verma:

Obesity is one and second is as like you know,

Ram Verma:

people age, these muscles become more floppy, more relaxed. So as

Ram Verma:

we age, it become more common. And also, some people have

Ram Verma:

narrow airway passes, so they inherit from the parents, if

Ram Verma:

their passes is narrow on the back of the throat, that make

Ram Verma:

them prone to have sleep apnea as well.

Anne Truong:

Okay, but is it preventable?

Ram Verma:

So it is a preventable in your sense that

Ram Verma:

if that is like the morphology they have, they cannot change

Ram Verma:

that one. But in terms of like, you know, they can control their

Ram Verma:

weight, that can help. They can sleep on the side, to some

Ram Verma:

extent, that can reduce the snoring and chances of sleep

Ram Verma:

apnea. But if someone like having symptoms, I suggest them

Ram Verma:

to, like, get it checked. And nowadays, very easy, like home

Ram Verma:

test can be done. And there are some device, some tracker comes

Ram Verma:

also, ring, smart watch. They can also give some idea if

Ram Verma:

someone is at risk of sleep apnea.

Anne Truong:

But you're saying it affects about 15% of men or

Anne Truong:

so. But if they feel tired, you, we know that a lot of things can

Anne Truong:

cause tiredness because it's hard for somebody to know. Do I

Anne Truong:

snore? Well, most people know I snore. Yeah, most people snore.

Anne Truong:

Unless you're sleeping with somebody, they're telling them

Anne Truong:

that you have pauses in your breathing while you're sleeping.

Anne Truong:

So should all men get a sleep study test? Because you

Anne Truong:

mentioned that you can get a sleep studies test at home now,

Anne Truong:

which I actually have one that I'm about to do for my husband

Anne Truong:

and for me for the sleep apnea test. So you think you have

Anne Truong:

sleep apnea, you can go to the doctor and get the sleep apnea

Anne Truong:

test either at home or in their office. So should everyone be

Anne Truong:

for all men be tested for this?

Ram Verma:

We don't recommend all men, but men like over 40,

Ram Verma:

and if they are having some fatigue, they are thinking their

Ram Verma:

quality of sleep is not good, feeling non restful when they

Ram Verma:

wake up in the morning and have been told snoring, breathing

Ram Verma:

pauses and having high blood pressure, or diabetes or any

Ram Verma:

depression, anxiety indicate that they might benefit with

Ram Verma:

sleep apnea testing.

Anne Truong:

Okay, got it. So, we talk about what sleep apnea

Anne Truong:

is. We talk about the risk factors. We talk about how

Anne Truong:

percentage of men, which is about 15% or so, and then is

Anne Truong:

that the only way to diagnose sleep apnea? Is the sleep test

Anne Truong:

or the home study test? Is there any other, when they see a

Anne Truong:

doctor that can work up for this?

Ram Verma:

So like these are the two methods, like home sleep

Ram Verma:

study or in lab sleep study. A home sleep study is more common

Ram Verma:

nowadays, with new technologies and more affordable and easy to

do. But clinically, if they are having symptoms, they can tell

Ram Verma:

the doctors or their providers and ask them to get like tested

for that.

All right. So you take the test, and then the doctor read, and

Anne Truong:

let's say you have sleep apnea. So what are the treatment for

Anne Truong:

sleep apnea?

Ram Verma:

So nowadays, there are different treatment options.

Ram Verma:

Most common is CPAP machine, BiPAP machine. What those

Ram Verma:

machine do? They push the air with gentle force with some mask

Ram Verma:

under the nose, or some mask covering nose and mouth both,

Ram Verma:

and that air come with force, and that air also has oxygen, so

Ram Verma:

that way it controls the sleep apnea. Sometime people run into

Ram Verma:

problems if a mask is not right, if sitting on the machine is not

Ram Verma:

right, then they are not able to use the CPAP machine. So in

Ram Verma:

those situation, I suggest them to see a sleep specialist if it

Ram Verma:

is not working, like no follow with like no different

Ram Verma:

specialist as well. There are other treatment option called

Ram Verma:

Inspire. So Inspire is an implant under the skin, and it

Ram Verma:

gives signal to a tongue to move forward, and when tongue move

Ram Verma:

forward, it open the passage on the back. So that is the implant

positional device for sleep apnea. They are like vibration

band, and that helps to avoid sleeping on the back. So that

Ram Verma:

band can be put on the stomach, then, or the neck. So that is

Ram Verma:

called positional device for sleep apnea, so that can also

Ram Verma:

help to some people.

Anne Truong:

Okay, so who do you determine who gets what? The

Anne Truong:

machine versus the Inspire versus dental prosthetics.

Ram Verma:

So CPAP is the gold standard treatment, so we start

Ram Verma:

with that. But if someone having lot of claustrophobia and they

Ram Verma:

want to go try dental device, that is fine too. So they can

Ram Verma:

get benefit from that. If their apnea is mild to moderate

Ram Verma:

category, not very bad, oxygen is not running low too much. So

Ram Verma:

they can benefit from dental device as well. About the

Ram Verma:

Inspire, someone who has moderate to severe degree of

Ram Verma:

sleep apnea, then they have tried CPAP, and they qualifying

Ram Verma:

for that, certain weight criteria, certain procedure to

Ram Verma:

look on the back of the throat, called DISE procedure. So if

Ram Verma:

they qualify for Inspire, they are good candidate. They can get

Ram Verma:

benefit from that as well.

Anne Truong:

Interesting. So do women get sleep apnea as common

Anne Truong:

as men?

Ram Verma:

It is less common in women, but after the menopause,

Ram Verma:

they tend to have almost similar prevalence for sleep apnea too.

Ram Verma:

And in women, the snoring is not that loud, just like men. So

Ram Verma:

some of the women after menopause, they are having

Ram Verma:

insomnia. It is presenting like insomnia, but they should also

Ram Verma:

get benefit from testing for sleep apnea as well, because the

Ram Verma:

symptoms are little bit subtle. They present like depression,

Ram Verma:

anxiety or insomnia. So symptoms are a little bit subtle, but

Ram Verma:

prevalence goes high after menopause.

Anne Truong:

Okay, well, I'm getting mine done. Only thing

Anne Truong:

is, I feel pretty good, so hopefully maybe I'll pass my

Anne Truong:

test. Who knows? But if my test is not normal, I'm definitely

Anne Truong:

calling you for this. So that's sleeping. Now, once you get on

Anne Truong:

the machine, you stay on it forever.

Ram Verma:

So most people stay like, you know, forever until,

Ram Verma:

unless they lose significant weight. So some people get

Ram Verma:

bariatric surgery. They lose 80, 100 pounds weight, and 60%, 70%

Ram Verma:

of them, they get off from the CPAP also. But some of them is

Ram Verma:

to benefit at lower setting on the CPAP. They still get good

Ram Verma:

quality of sleep with CPAP. So for them, maybe it is from the

Ram Verma:

morphology, how the bone are aligned, not only the weight.

Anne Truong:

What you're saying, which I didn't know, was that if

Anne Truong:

you lose weight, you can actually reverse your sleep

Anne Truong:

apnea?

Ram Verma:

In some percentage.

Anne Truong:

How much percent?

Ram Verma:

After people get bariatric surgery, 60% to 70% of

Ram Verma:

people, they get off from a machine. Their sleep apnea get

Ram Verma:

reversed.

Anne Truong:

Okay, so if you lose weight, then that will

Anne Truong:

help. What else can you do to not be on the machine? Is there

Anne Truong:

anything else you can do besides just losing weight if you don't

Anne Truong:

want to be on the sleep apnea machine?

Ram Verma:

Yes. So there are other treatment option. Like I

Ram Verma:

mentioned, Inspire. So Inspire is without any mask. It keep

moving the tongue in the night when people are sleeping and the

Ram Verma:

movement of tongue is not painful.

Anne Truong:

Yeah, like natural therapy, instead of, like a

exercise or anything that they can do naturally?

on their side, that is helpful. Sometimes people elevate their

head, just leave it and they are sleeping better. So those are

other things they can do. But I used to recommend to get it

Ram Verma:

treated, because when they go into deeper sleep, these muscle

Ram Verma:

relax, and in the deeper sleep, we want our muscle relax, and

Ram Verma:

that's how it happens. So that's why the treatment is necessary.

Anne Truong:

So the end point is, if you do the test and it

Anne Truong:

showed you have sleep apnea, you should use the machine and try

Anne Truong:

to lose some weight. Obviously, losing some weight, will

Anne Truong:

probably help guess the parameter of the machine as

Anne Truong:

well, too, right?

Ram Verma:

Yeah, yep. And not only the machine, not only the

Ram Verma:

machine is option. Like other options, like dental device

Ram Verma:

putting into the mouth that is one option. Positional device

Ram Verma:

for sleep apnea and inspire, those are the options without

Ram Verma:

any CPAP. And there is a another device called iNap that, like

Ram Verma:

people put into the mouth, and it create negative suction in

Ram Verma:

the oral cavity, so keeping the tongue sucked up on the front so

Ram Verma:

they keep breathing from the nose. So there are some other

Ram Verma:

device also. So I see patients. Lot of them, they are doing good

Ram Verma:

on the CPAP. But some who are not doing good on the CPAP, I

Ram Verma:

try to use other alternative method, without any mask.

Anne Truong:

So that's good. But the bottom line, if you're going

Anne Truong:

to use the machine, you use it forever?

Ram Verma:

Most of the time, yes.

Anne Truong:

Okay, I see the machine. One of my friend has

Anne Truong:

one. It's very small and quiet now, not like, you know, it was

Anne Truong:

like 27 years ago when I started medicine. The machine then look

Anne Truong:

like a computer, and it makes so much noises, it was incredible.

Anne Truong:

I think that was what the main thing was, you know, I want our

Anne Truong:

listeners how to know if they have sleep apnea. So what

Anne Truong:

happens if you don't get treated with sleep apnea? What happens?

Ram Verma:

So sleep apnea causes intermittent low oxygen in the

Ram Verma:

body, and oxygen is very important things for our

Ram Verma:

survival. If we don't get oxygen for five minutes, we are dead.

Ram Verma:

So the brain keep looking the level of oxygen. If there is a

Ram Verma:

drop of 3% 4% then brain keep getting jerk, keep getting

Ram Verma:

arousal, and that's how it break the rhythm of sleep, so people

Ram Verma:

don't get quality sleep.And in that way, because of that, they

Ram Verma:

are not getting restful sleep, not getting quality sleep. So

Ram Verma:

that is affecting overall health. So affecting the ability

Ram Verma:

of brain to clear the toxins. And when toxin accumulate, then

Ram Verma:

they people develop dementia and depression, anxiety and then lot

Ram Verma:

of problem to the heart and stroke. As you mentioned, sexual

Ram Verma:

health also get affected because of these things, not getting

Ram Verma:

good quality of sleep, not feeling rested, and having low

Ram Verma:

hormones. So those affect sexual health as well.

Anne Truong:

Okay, great. So to summarize, get your test and get

Anne Truong:

treated. It's very unintrusive. And that if you don't you're

Anne Truong:

going to be at risk for dementia, and not to mention,

Anne Truong:

making your ED worse because your body is not getting enough

Anne Truong:

oxygen while you sleep. When you're not getting enough oxygen

Anne Truong:

while you sleep, it will affect everything. Your brain, your

Anne Truong:

heart, your sexual function as well, and you continue feeling

Anne Truong:
