doctor who treated over 7000 men with ED. In today's episode,

we're going to talk about, how can you tell whether your

Speaker:

erectile dysfunction is psychological versus is it a

Speaker:

physical cause? I get that question a lot, especially in my

Speaker:

patient who come by to see me, and they want me to

Speaker:

differentiate whether their ED is either physical or is it more

Speaker:

psychological? So let's kind of dive into this.

Speaker:

What are the psychological causes of ED? Number one, it

Speaker:

could be stress and anxiety, like performance anxiety, fear

Speaker:

of intimacy, or past trauma can contribute to ED. Depression can

Speaker:

both affect sexual desire and erectile function. Relationship

Speaker:

issues such as poor communication between the

Speaker:

partners, unresolved conflict or even emotional distance. The

Speaker:

fourth one is low self esteem, often due to past episode of ED

Speaker:

or maybe unrelated issues. If the symptoms of ED, for

Speaker:

psychological ED, can be that ED may occur in a specific

Speaker:

situation or with certain partners, but with other

Speaker:

partners it may not occur. And that the symptoms are variable.

Speaker:

It's inconsistent and it's not predictable. It's known by the

Speaker:

inconsistent ability to achieve or maintain erection. But it's

Speaker:

also maybe different in circumstances, also different

Speaker:

with partner. The number one thing is that you're able to

Speaker:

achieve nighttime erection, and you're able to achieve

Speaker:

masturbation. So you can get an erection with masturbation, and

Speaker:

you can get an erection with morning wood.

Speaker:

Now, what are some of the physical causes of ED there are

Speaker:

many. Let's just kind of hit on the major point. Number one is

Speaker:

that if you're smoking, you're going to get ED. That's what

Speaker:

number one physical cause. And if you're watching porn, then

Speaker:

you're going to have ED, period. And then the other major

Speaker:

physical issue is vascular issues like heart disease or

Speaker:

diabetes, and then issues such as low testosterone or chronic

Speaker:

conditions such as obesity and certain medication may cause it.

Speaker:

And also neurological disorders such as multiple sclerosis, such

Speaker:

as the spinal cord injury, but the number one thing is that

Speaker:

vascular condition. So if you have heart disease, heart

Speaker:

disease mainly happen from either high cholesterol or high

Speaker:

blood pressure, and also, if you're drinking more than 14

Speaker:

drinks a week, that can contribute to ED as well.

Speaker:

ED from physical causes are gradual. It developed over time,

Speaker:

developed over years, and it's a consistent symptom, unlike the

Speaker:

psychological ED which is inconsistent symptom. The

Speaker:

physical cause of ED is consistent symptoms like

Speaker:

persistent inability to achieve or maintain the erection across

Speaker:

all situation, across all partners, across all

Speaker:

environments, and that the physical sign may include, maybe

Speaker:

atrophy or the penis getting smaller, or there may be bending

Speaker:

of the penis as well too. So the diagnostic approach to tell

Speaker:

difference between psychological and physical ED, the doctor

Speaker:

takes a very thorough history of all your medication, all your

Speaker:

chronic condition, chronic condition to consider like high

Speaker:

blood pressure, diabetes, high cholesterol, and your weight,

Speaker:

your heart rate, all your medication that you take, and

Speaker:

then your past medical history that you've had in the past. And

Speaker:

then the physical examination is a test to look for your blood

Speaker:

flow like, how good is your pulses? What is your heart like,

Speaker:

your lungs like? And making sure to check that you don't have any

Speaker:

testicle abnormality or any curvature of the penis, or any

Speaker:

restrictions, any even atrophy, and also evaluating the

Speaker:

integrity the pelvic muscles.

Speaker:

Now on top of that, we look at blood tests. So doctors look at

Speaker:

blood tests like your hormone level, looking at your blood

Speaker:

sugar, looking at your blood level to see if you're anemic,

Speaker:

looking at your kidney function, your liver function, and your

Speaker:

testosterone level, your total, your free and your sex hormone

Speaker:

binding globulin data as well. And then, if necessary, do a

Speaker:

penile doppler ultrasound, as an ultrasound, where we look at the

Speaker:

blood vessels of the penis to determine how much blood flow is

Speaker:

going into the penis from the artery and how much distension

Speaker:

of the veins. And then, now there is a ring that you can

time or morning erection that you may not be aware of. And of

course, as doctors, we also assess to see what your stress

So you account for all this, and with the thorough history, you

can somewhat differentiate psychological and physical

Speaker:

causes. And then, once this is determined, a treatment is

Speaker:

tailored based upon the identified causes. Always treat

Speaker:

the cause of your ED rather than just taking an ED medication.

Speaker:

Because when you take an ED medication, you're just treating

Speaker:

the symptoms. You're not treating the cause of ED. If

Speaker:

you're taking ED medication, it's like putting a band aid on

Speaker:

an infected wound. You're just covering it up because you're

Speaker:

not treating the causes, which could be diabetes, high blood

Speaker:

pressure. Maybe it's the smoking, maybe this drinking a

Speaker:

lot, and maybe it's the overweight then. So if you're

Speaker:

not treating the cause of the symptoms, then your ED will only

Speaker:

get worse over time, even with ED medication. So I hear men

Speaker:

say, "Well, I take Viagra, and it works very well, but what's

Speaker:

going to happen in a year from now, three years from now, it's

Speaker:

not going to work very well, because the cause of why you

Speaker:

have ED in the first place have never been addressed. Just like,

Speaker:

if you have a tooth that is hurting, would you just brush it

Speaker:

and just soak it with just mouthwash, rather than trying to

Speaker:

go see the dentist and find out what's going on. So look for the

Speaker:

root cause of the problem and treat it. You treat it early,

Speaker:

you're gonna have total reversal of your ED and sometimes it's

Speaker:

just as little as changing your diet, and sometimes it's just as

Speaker:

little as maybe you need to lose a little bit of weight. And if

Speaker:

you start early, then you're gonna have more remarkable

Speaker:
